Saline Memorial's dedication to your health doesn't end at the hospital or physician's office. There may be a time when your require care in the home. Saline Memorial has been providing Home Health services to Saline County and surrounding areas for over 28 years. The Saline Memorial Home Health team is a “healing link” between the hospital and your home. 

Our team provides and coordinates the care and/or therapy ordered by your doctor. Our staff develops a personalized plan for each individual patient that includes the services you will receive to improve and maintain your physical, mental, and social well being.

• Speech Language Pathology Services

Rehabilitation services also help restore functions that may have been impaired by illness or surgery. Home Health Care often involves learning about a newly diagnosed disease such as diabetes, heart failure or pulmonary disease. The Saline Memorial Home Health team guides patients in blending self-management skills into their daily lifestyle.

INITIATING HOME HEALTH SERVICES

Any individual or physician may request services by calling Saline Memorial Home Health Services. Following a request for Home Health Care, the patient’s physician will be contacted for approval of services. You can initiate services by calling during office hours: 8:00am to 4:30pm, Monday – Friday. Home Health is covered by Medicare, Medicaid and most commercial insurance, HMO’s and PPO’s.

PERSONAL CARE

Saline Memorial Home Health offers a personal care program to the community. Certified Nursing Assistants may provide help with bathing, dressing, grooming and other activities of daily living in the home. These services may be funded by private insurance, Medicaid, or private pay.
