Basic Benefits Program

Note: Unless other requirements are stated – All full time and part time employees become eligible for benefits on the first day of the month following 30 days of continuous service.

Group Health Insurance

Medical coverage includes physician services, hospitalization, and prescription drugs. A 20% employee discount is applied to hospital facility charges performed at Saline Memorial Hospital after deductible.

Group Dental Plan

Our dental coverage includes: Diagnostic and Preventative, Basic Restorative and Major Restorative services. Also a Child Orthodontia Rider for children up to age 19.

Group Vision Plan

Optical coverage focuses on eyes and overall wellness. The plan includes eye exam and prescription glasses or contact lenses.

Life Insurance/Accidental Death and Dismemberment (AD&D) Plan

To help ensure the financial security of those who depend upon you, we purchase coverage in the amount of one times your annual salary. Managers are provided two times annual salary.

Voluntary Term Life Insurance/Accidental Death and Dismemberment (AD&D) Plan

Optional life insurance for employee, spouse, and dependent children.

Permanent Voluntary Term Life Insurance Plan

Optional life insurance for employee, spouse, and dependent children.

Short Term Disability (STD)

Protects your income for up to six months in case you are unable to work because of non-work related illness or accidental injury. The plan pays 60% of your weekly income. Depending on the option you choose, benefits begin on the 15th or the 31st day after the injury or illness. .

Long Term Disability (LTD)

Protects your income after 180 days of disability. The plan pays 60% of monthly base income up to $10,000 per month.
Cancer Plan Policy pays you directly so you can use the additional dollars to pay for expenses not covered by medical plan or indirect costs such as: loss of wages, travel expenses to & from treatment centers, lodging and meals and child care. A wellness benefit is included.

Group Critical Illness

Provides a cash benefit at the diagnosis of a covered illness to be used for unforeseen expenses not covered by health insurance. A wellness benefit is included.

Accident Insurance

Provides a cash benefit based on the type of injury or covered incident sustained. These dollars can be used for unforeseen expenses not covered by health insurance.

Flexible Spending Accounts (Section 125)

Section 125 of the Internal Revenue Code allows the opportunity to pay for certain expenses on a pre-tax basis rather than on after-tax basis. Health Care Spending Account is a pre-tax account for qualifying out-of-pocket health, dental, and vision expenses. Dependent Care Spending is a pre-tax account for qualifying out-of-pocket dependent care expenses.

401k Retirement Plan with Employer Matching Dollars

After one year of continuous service and 1000 hours worked, SMH will begin match of 401k contributions up to 5%. Must be at least 21 years of age. The plan gives you investment options so you can select the investment strategy that makes the most sense for you and the flexibility to change how you save and invest that money.
