The goal of SMH
Hospice care is to achieve the best quality of life for
patients and there families dealing with life-threatening
illnesses. Caring for the whole patient-physically,
spiritually and psychologically is the basis of SMH hospice.
To ensure our patients receive the essential medical and
emotional care during this most difficult time in life, SMH
Hospice plans to extend the program to include a 16-bed
inpatient facility located in Bryant. The facility will
serve a growing monthly patient census of approximately
55-75. Plans for the new in-patient facility include a
sunroom, chapel, family room, in-patient beds, dining room
as well as staff facilities.
Hospice care is provided by an interdisciplinary team which
includes the physician (including the Hospice Medical
Director), nurses, social worker, chaplain, patient care
technician, hospice volunteers and bereavement coordinator.
Care can be provided in the patient’s home, family
residence, nursing home or retirement home. In addition,
short-term hospice inpatient care is also available at SMH. |
