company's criteria of medical necessity. You may contact your
insurance company for additional information. |

Saline Memorial Hospital under a contract. |
|
| Q4 |
Why does it take so long to receive a monthly statement? |

Patients
who have insurance through a HMO that contracts with the
hospital are unable to send out monthly statements until the
claim is either paid with a balance due or considered denied. |

|
| Q5 |
Who is responsible for obtaining a pre-authorization number? |
|
| A5. |
Once
insurance is verified, we will ask if you have obtained the
necessary pre-certification of referrals as required by most
insurers. If you have not done so, we will assist you.
However, please understand that most insurance companies
require advance notification before you enter the hospital or
receive hospital care. In non-emergencies, we urge you to take
care of these matters before you come to the hospital.
