Memorial Hospital is an active community partner participating

service designed to sustain our patients and their family
members during and after hospitalization.

The CM
Department offers continuous care beginning with
pre-admission through post-discharge. Caseworkers interview
patients and/or family members once their specific needs are
identified by nursing assessments.

Hospital are professionals who are trained to accommodate
each individual situation with compassion and understanding.
